The form is for NU faculty and staff, as well as students who need media materials for classroom presentations.

You must fill out a separate form for each date needed. Please submit this form at least one week in advance of the date needed to allow library staff ample time to process your request.

COPYRIGHT: Copyright law restricts use of "home use only" materials to classroom instructional showings as part of a course with an instructor present or for research or class preparation by faculty. Other materials may be restricted by licensing and/or contractual agreements.

USE LIMITATIONS: Media items may be borrowed for 1 week, unless they are on reserve for a course. Faculty may take reserve items out of the library to show in class. These materials should be returned promptly by the next day, as they may be needed for other courses.
